Confidentiality Agreement
Data Protection
Medway Youth Trust is formally registered with the Information Commissioners Office and is compliant with all requirements of Data Protection legislation. Medway Youth Trust operates and maintains a database for the delivery of Connexions services. It is a government requirement that we maintain this and we operate it within very strict guidelines using Government approved software. Our staff receive training when they join us and also receive ongoing refresh training to ensure we all use the software properly and effectively.
When we do share or receive information, we do so within specific data sharing protocols which again comply with legislation and good practice.

Confidentiality Agreement
We provide a confidential service for your child. This means we will not usually share information with you unless of course there is a legal requirement or duty to pass on information, or if there is a risk of serious harm or threat to life.
When your child receives Connexions support from us, we ask them to sign a confidentiality agreement. It gives name, address, telephone number, date of birth and last or current school/college. The following words are on the agreement:
“The information you have given on this form will be held in paper format and on our secure client database. It is covered by the Data Protection Act 1998. This means that you have the right to ask us to correct any inaccuracies and see any information that is kept about you, for which we may charge a fee. This information will be shared within Connexions Medway and Medway Youth Trust.
You also have the right to say whether this or other information about you is shared with other organisations. We will be able to help you more effectively if you allow us to share your information with organisations such as schools, colleges, training organisations and employers when you or your Personal Adviser think it would be of help to you. We only share information that is needed and in your interest.
Your Personal Adviser may ask you to complete a consent form to give us permission to pass on your information. However,”.
We then ask your child to tick a box which gives or refuses consent.
